One sheet per child per week keeps the record unambiguous — a shared room-wide sheet makes it harder to prove exactly who dropped off or picked up a specific child on a specific day.

A blank week (nothing in the exceptions line) is a good week — this line exists so an unusual pickup doesn’t just live in someone’s memory.
How to use this template
- Print one copy per child at the start of each week; keep it on a clipboard in the child’s room or at the front desk where drop-off and pick-up happen.
- Have the adult dropping off or picking up sign or initial in person, at the time it happens — not filled in afterward from memory.
- Cross-check any unfamiliar adult against Section 2’s authorized-pickup list before releasing the child, and ask for photo ID if your center’s policy calls for it.
- Keep completed weekly sheets with your center’s attendance records. How long you need to keep them, and whether initials or a full signature are required, depends on your state’s child care licensing rules — check with your state licensing agency rather than assuming a single national rule.
