Consent given on this form can be withdrawn at any time, for any of the choices above, by contacting the organisation using the details in Section 1. Withdrawing consent does not affect anything already done on the basis of consent given earlier, and will not be held against the child or family.

How to use this template

  • Fill in Section 4 first, in plain language, every time you reuse this form for a new activity or request — that description is what keeps the rest of the consent specific and informed rather than a blanket “yes.”
  • Keep each consent choice (participation, photo/media use, information sharing, emergency medical treatment) separate and separately answerable — do not bundle them into a single sign-here box.
  • Omit the alternate emergency contact or the whole health information section when the activity genuinely does not need them; asking for information you will not use is its own problem.
  • Keep a copy of each signed form on file according to the practice you state in Section 11, and log any withdrawal of consent against the relevant child’s record.
  • Review the wording against your own jurisdiction’s data-protection and child-safeguarding rules before relying on it — this is a general-purpose starting point, not legal advice.
