How to Use This Template
- Fill in Section 1 and the Section 2 snapshot before the meeting, so the conversation starts from evidence rather than memory.
- Complete Sections 3–6 during the conference; keep entries factual and specific, not vague impressions like “doing well.”
- Give every goal a named owner and a target date — a goal with neither is an aspiration, not a plan.
- Give the parent/guardian a copy of the completed record. A shared, written summary is what keeps everyone working from the same agreement between now and the follow-up date.
- This is a routine conference form, not a special-education annual review. Where a student has an IEP (US) or an EHCP/SEN support plan (UK and similar systems), that plan’s own annual review meeting is a separate, legally required process with its own required content — this template does not replace it. No general law requires schools to hold routine parent-teacher conferences or dictates their format; check your school’s or district’s own policy.
- If anything discussed becomes a safeguarding concern, follow your organisation’s safeguarding procedure and record it in the appropriate child-protection file, not only on this form.
- Keep completed conference records on file for as long as your organisation’s student-record retention policy requires — this varies by school, district, and country, so this template does not state a specific retention period.
