Fill in the student and contact details once, then add one row to the log table every time you call, email, meet, or message a parent/guardian about this student. Keep it in the student’s file so any staff member can see the communication history without relying on memory or scattered notes.
This log covers ongoing contact across the whole term or year. Section 4: How to Complete an Entry
Each row below is one contact. Use short, factual language — write what was said and agreed, not a general impression:
- Date — the date the contact actually happened.
- Method — call, email, in-person meeting, app/portal message, written note home, or other.
- Initiated by — write “School” or “Parent” depending on who reached out first.
- Staff member — who made or received the contact, so a colleague covering the class later knows who to ask.
- Topic & summary of discussion — what the contact was about and what was actually said or agreed — specific enough that someone who wasn’t there understands what happened.
- Follow-up needed — write “None,” or note the action and who owns it (e.g. “Yes — call back Friday, J. Smith”).

If a contact touches a safeguarding concern, follow your organisation’s safeguarding procedure and record it in the appropriate child-protection file — this log is a communication record, not a substitute for that. Keep entries to what is needed to track the contact itself; avoid recording sensitive personal detail here that belongs in a different, more restricted file.

How to use this template
- Complete Sections 1–2 once, when you open the log for a student at the start of the school year or term.
- Add Section 3 only if a second parent/guardian is on file — skip it otherwise.
- Add one row to the Section 5 table every time you have a parent/guardian contact about this student — right after the contact, while it’s still fresh, not from memory days later.
- Keep the summary factual and specific: “explained the missed-homework pattern this week and agreed to a daily check-in note” is useful later; “talked about homework” is not.
- Give every “Yes” follow-up a named owner and, where possible, a date — an open follow-up with no owner tends to get lost.
- This log is a record of contacts, not a case file — a safeguarding concern, a formal complaint, or anything requiring a separate incident report should be filed through your organisation’s own process, with only a brief reference logged here if relevant.
- Keep the completed log in the student’s file for as long as your organisation’s own retention policy requires; this varies by school, district, and country, so this template does not state a specific period.
