How to use this template

  • Open one student grade record sheet per pupil per school year, and keep it with that pupil’s file — do not share a sheet across pupils or across years.
  • Before entering a single mark, fill in Section 2 with the marking scale in use and the pass mark for that scale; a column of bare numbers is unreadable a year later without it.
  • Record every mark under the assessment period it belongs to, together with the weight or coefficient it carries toward that period’s average.
  • Add a short note next to a mark only when it explains something the number alone cannot — a missed assessment, a retake, a moderation adjustment.
  • Recalculate the running average as each new mark is added rather than only at the end of the period, so an entry error is caught early.
  • Restate the weighting and the scale in the end-of-year summary (Section 6), even though both are already recorded in Section 2 — a summary read on its own should still be checkable by anyone.
  • If the school changes marking scale mid-year, note the change and the conversion used rather than mixing two scales silently on one sheet.

Identifies whose record this is, and for which class and year. Complete this section before any mark is entered.


Section 2: Marking scale in use

This is the section most grade sheets skip, and the one that makes a sheet unreadable later: a bare mark means nothing without the scale and the pass mark it was set against. Fill this in once, at the top, before recording any mark.

Restate the weighting and the scale here even though they already appear in Section 2 — this section is what most people read a year later, and it should be checkable without the rest of the sheet.
